The State of Ohio requires that certain information be made available to women seeking abortions at least 24 hours prior to their procedure. This information will eventually be available online on a State of Ohio website. Until then, you may obtain this information through our facility or by clicking the link below.

Consent Form #1
-- "Woman's Right To Know Documentation"

You will need to read this form and acknowledge that you have read the form to make an appointment. This is required to verify that you were given this information at least 24 hours before the procedure.

The State of Ohio does not have these materials online or in written form as of the effective date of the bill.

Consent Form #2: A consent form which certifies that you are terminating your pregnancy freely and without coercion. It details the possible risks and complications associated with the abortion procedure and anesthesia type; the gestational age of the fetus documented by ultrasound; the name of the physician/facility where the termination will take place and all other associated risks of a voluntary/elective surgical procedure.

You will receive this form at the Akron Women’s Medical Group the day of your appointment.
